Compassion, as an indicator for quality care, is highly valued by patients and healthcare professionals. Compassionate care is considered a moral dimension of nursing practice and an essential component of high quality care. This study aimed to answer these questions: (1) What are the facilitators and barriers of providing compassionate nursing care in the clinical setting? (2) Which strategies do nurses use to provide compassionate care? (3) What is the specific model of compassionate care for the nursing context? A grounded (...) theory approach was used in this study. A total of 21 nurses working in diverse clinical settings participated in the study. Purposive and theoretical sampling was used to select the participants. Data were collected by in-depth face to face interviews and analyzed by the constant comparative method. Ethical approval was gained from the Ethical Review Board of Tabriz University of Medical sciences. The analysis resulted in the development of three main themes: (a) contextual factors affecting compassionate care, (b) the compassionate care actions, and (c) the consequences of compassionate care. The main dimensions of compassionate care are demonstrated in a Compassionate Nursing Care Model. Nurses’ ability on providing compassionate care is influenced by individual and organizational factors that may facilitate or inhibit this type of care. Leadership and nurse managers should remove the barriers which diminish the nurses’ ability to provide compassionate care and support them to engage in compassionate care programs. Identifying and recruiting compassionate nurses, developing their compassionate capacity, and providing role models of compassion could improve the flourishing of person-centered and compassionate care in clinical settings. Background: Compassionate care is an international priority of healthcare professionals. There is little understanding about how workplace issues impact provision of compassionate care in nursing practice. Therefore, it is important to address the workplace issues and organizational factors which may hinder compassionate care delivery within nursing practice. Objective: The aim of this study was to explore workplace and organizational barriers to compassionate care from the nurses’ perspective. Research design: The study used a qualitative exploratory design, and data were analyzed by (...) conventional content analysis. Participants and research context: A total of 15 nurses working in different fields of nursing were recruited from four hospitals at northwest of Iran. Participants were selected by purposive sampling. Semistructured interviews were conducted for data collection. Ethical consideration: Ethical approval of this study was gained from the Ethical Review Board of Tabriz University of Medical Sciences. Findings: The main theme which emerged from data analysis was “unsupportive organizational culture.” This theme had two main categories including “excessive workload alongside inadequate staffing” and “the lack of value on compassionate care.” Discussion: Organizational barriers to development of compassionate in clinical practice were identified in this study. A closer examination of these barriers is required to move compassionate practice from an individual responsibility to a collective responsibility that is owned and shared by organizations. Conclusion: For compassionate care to flourish, policy makers, managers, and healthcare providers must foster an organizational atmosphere conducive to compassionate care. (shrink)

Medical ethics is a practical key element in the medical curriculum. Movies can be used as an effective and innovative way to involve students in discussions and reflections on ethical issues. This study aimed to evaluate the appropriateness of medical movies as a tool in medical ethics education. During the last teaching session of the medical ethics courses, the movie “Extreme Measures” was shown to the medical students. The present study is a descriptive-analytical study of 302 students’ manuscripts. The quantitative (...) content analysis of the students’ manuscripts was carried out using a checklist that contained 11 titles and 75 subtitles. Each student’s checklist was compared to the student’s final exam score. The most recognized issue was justice, and the least recognized one was confidentiality and privacy. Nonmaleficence, respecting autonomy and informed consent, honor and integrity, end of life, duty and responsibility, conflict of interest, beneficence, relationship with colleagues and altruism were located in-between. There was a relationship between students’ final exam scores and their manuscripts’ scores. According to our findings, relevant movies with standard categorization can be used as a useful method for medical ethics education and assessment. (shrink)

As artificial intelligence technologies become increasingly prominent in our daily lives, media coverage of the ethical considerations of these technologies has followed suit. Since previous research has shown that media coverage can drive public discourse about novel technologies, studying how the ethical issues of AI are portrayed in the media may lead to greater insight into the potential ramifications of this public discourse, particularly with regard to development and regulation of AI. This paper expands upon previous research by systematically analyzing (...) and categorizing the media portrayal of the ethical issues of AI to better understand how media coverage of these issues may shape public debate about AI. Our results suggest that the media has a fairly realistic and practical focus in its coverage of the ethics of AI, but that the coverage is still shallow. A multifaceted approach to handling the social, ethical and policy issues of AI technology is needed, including increasing the accessibility of correct information to the public in the form of fact sheets and ethical value statements on trusted webpages, collaboration and inclusion of ethics and AI experts in both research and public debate, and consistent government policies or regulatory frameworks for AI technology. Today, the US pursues the global capture of data (understood as a significant engine of growth) by way of bi- and plurilateral trade agreements. However, the project of securing the global free flow of data has been pursued ever since the dawn of digital telecommunication in the 1960s and the US has made significant legal efforts to institutionalise it. These efforts have two phases: In the first 1970s and 80s “freedom of information” phase, the legal justification (and contestation) of the (...) global free flow of data hinged on imagining data as information, and its exchange as a practice of liberty. The second phase began in the late 1990s and continues today. In this phase, the free flow of data is aligned with a free-trade agenda in the context of first e-commerce and, starting in the 2000s, through attempts at creating a global public domain of personal data for the platform economy. The global free flow of data is an intrinsic aspect of informational capitalism. Assuming a constitutive, but not commanding role for law in informational capitalism, we conclude that the US attempt at ensuring free flow for its informational corporations is neither an entirely contingent nor a necessary outcome. A concretização do direito à saúde deve ser viabilizada pelo Estado, com foco na efetivação das políticas públicas para garantia da implantação e funcionamento dos serviços demandados pelos respectivos grupos populacionais. Destarte, essa escrita objetivou analisar como o direito à saúde é contemplado em comunidades quilombolas de uma região geográfica baiana. Metodologicamente enquadra-se como de abordagem qualitativa, com informações obtidas por meio de entrevistas estruturadas (sobre a infraestrutura e/ou serviços públicos de saúde) desenvolvidas com as representações das Associações de Moradores (...) de 14 quilombos da região geográfica de Guanambi/Bahia durante o ano de 2016. Os achados indicam que uma comunidade possuía estrutura física específica para atendimentos básicos em saúde, no entanto, estava desativada. Três comunidades não tinham cobertura do Programa de Agentes Comunitários de Saúde (PACS). Entre as 11 que indicaram cobertura do Programa, em sete as(os) profissionais não residiam no território do quilombo. Diante da ampla ausência de infraestrutura e recorrente inadequação na prestação de serviços do PACS, evidencia-se a permanente negligência ao direito à saúde nesta população quilombola. (shrink)

MayAccording to one tradition in the theory of fiction, there is a kind of fantasy whose function is to invite the reader to "acknowledge the possibility of a different reality."1 In this essay I want to ask whether Lewis Carroll's Alice books fit into this category; that is, I want to explore the possibility that Alice's Adventures in Wonderland and Through the (...) Looking-Glass are not the pure nonsense that many readers take them to be. In considering the Alice books as fantasy offering a view of a "different reality" I do not deny that there are in these stories some mimetic elements alongside the fantastic—representations of things going on somewhere, somehow, in Victorian society—in which case the "different reality" presented will not be so terribly different as to be unrecognizable. For example, the Mad Tea Party in Alice's Adventures in Wonderland is not only an act of fantasy but also an act of mimesis. It certainly imitates the real tea parties often held throughout Victorian England—if caricature is a form of imitation—but it may also mime those mock tea parties held in English asylums whose goal was to model sane-like behavior to the inmates.2 An analogous question can be asked about Carroll's books in general: In addition to the component of fantasy, are they mimetic of Victorian culture itself, or merely of the Victorian conception of madness? Michel Foucault has argued that, while the very idea of la folie is an historical construct, the dialectical relationship between reason and unreason, with each pole parasitic on the other, is a structural constant. Carroll's Alice books produce images that are discharged in the confused nineteenth-century landscape where these two opposites meet. In that respect, his books are not so different from many of the more standard novels written in mid-century England. In this essay I [End Page 79] will explore features of this landscape, using a theoretical framework that has been synthesized from a number of related claims in the work of the linguistic philosopher Ludwig Wittgenstein. In addition to my long-range aim of urging further study of Wittgenstein's work by literary critics, one of my specific goals is to show that the application of Wittgensteinian methods to the works of Lewis Carroll reveals something about the hidden logic of Carroll's stories, and conversely that this procedure tests some of Wittgenstein's intuitions concerning language. Yet another goal is to suggest that the structure of the discourse of domesticity as reflected in many mid-nineteenth-century novels is curiously similar to the structure of discourse in Wonderland and Looking-Glass Land—that the language-games played at these sites are at the same time functional and dysfunctional in much the same way. That may seem like a lot to attempt in a few pages,... but to paraphrase Humpty-Dumpty, when I ask words to do that much extra work, I pay them overtime.In 1965 the philosopher George Pitcher published an essay, "Wittgenstein, Nonsense, and Lewis Carroll," in which he sought to demonstrate "the remarkable extent and depth of affinity between these two great writers with respect to nonsense." In addition, he showed that "the very same confusions with which Wittgenstein charged philosophers were deliberately employed by Carroll for comic effect," and that many of the examples that Wittgenstein used to illustrate his points "resemble, in varying degrees, examples that are found in the works of Carroll."3 While I agree with Pitcher's observation that the examples of nonsense that Wittgenstein offers are very similar to a primary strain of nonsense that runs through the Alice books, in the first section I intend to go beyond that observation and to apply some of Wittgenstein's tools to Carroll's work and show that Wittgenstein's work can be used to reveal features of literary narrative that might otherwise be obscured, and in the second section, to apply these tools to the works of fiction this side of the Looking-Glass.IWittgenstein says that a language is a "language-game." The process of agricultural unification will dominate the German scene for the next few years. At this stage, however, analysis and forecasts are hampered by the considerable problem concerning the viability of East German agriculture in a market economy and by the absence of reliable data in the German Democratic Republic (GDR). This can only be understood in terms of the agricultural situation in the GDR before unification, which is discussed in the first section. The political and economic incentives for (...) German unification are described in the next section of this paper, focusing especially on the integration of the GDR into CAP. The third section considers some critical structural, legal, social, and economic constraints in the process of integrating the two agricultural systems. Finally, the implications for German agriculture as well as the challenges for the CAP will be addressed. Data were collected from interviews with key agricultural policy makers, farm organizations, commodity groups, consumer groups, and agricultural experts, as well as interviewing East German farmers. Additional data were compiled from government statistics, EC publications, and press reports. The conclusions reached in this research show the urgent need for the German government to reconsider the objectives and rationale for structural policies in agriculture, and point out that the new Germany, itself full of paradoxes and uncertainty, stands at the center of European agricultural disorder. This paper aims to analyze the deictic representations of person in the British and American media discourse, mostly focusing on such genres and subgenres as newspaper articles, interviews, letters to editors, opinions, headlines and advertisements. For this purpose, we wish to introduce a theoretical framework for the study and then we hope to present certain ways in which deictic expressions represent person. Theoretical framework for our study is based upon the socio-cognitive approach, which gives priority to individual practices and subjectivity (...) over social practices in discourse as well as the definition of discourse based on knowledge. The reason why we focus on the media discourse is obvious. The high degree of personalized conceptualization and contextualization dominated by a large number of subjective factors prevail in the above-mentioned genres and subgenres of media discourse. This article discusses findings from a mixed method literature review that investigated cancer patients’ perceptions of what constitutes a good nurse. To find pertinent articles, we conducted a systematic key word search of five journal databases (1998—2008). The application of carefully constructed inclusion criteria and critical appraisal identified 12 relevant articles. According to the patients, good nurses were shown to be characterized by specific, but inter-related, attitudes, skills and knowledge; they engage in person-to-person relationships, respect the uniqueness of patients, and (...) provide support. Professional and trained skills as well as broad and specific nursing and non-nursing knowledge are important. The analysis revealed that these characteristics nurtured patient well-being, which manifests as optimism, trust, hope, support, confirmation, safety and comfort. Cancer patients’ perceptions of what constitutes a good nurse represent an important source of knowledge that will enable the development of more comprehensive and practice-based views on good nursing care for such patients. These perceptions help us to understand how nurses effectively make a difference in cancer patient care. (shrink)

Dung’s (1995) argumentation framework takes as input two abstract entities: a set of arguments and a binary relation encoding attacks between these arguments. It returns acceptable sets of arguments, called extensions, w.r.t. a given semantics. While the abstract nature of this setting is seen as a great advantage, it induces a big gap with the application that it is used to. This raises some questions about the compatibility of the setting with a logical formalism (i.e., whether it is possible to (...) instantiate it properly from a logical knowledge base), and about the significance of the various semantics in the application context. In this paper we tackle the above questions. We first propose to fill in the previous gap by extending Dung’s (1995) framework. The idea is to consider all the ingredients involved in an argumentation process. We start with the notion of an abstract monotonic logic which consists of a language (defining the formulas) and a consequence operator. We show how to build, in a systematic way, arguments from a knowledge base formalised in such a logic. We then recall some basic postulates that any instantiation should satisfy. We study how to choose an attack relation so that the instantiation satisfies the postulates. We show that symmetric attack relations are generally not suitable. However, we identify at least one ‘appropriate’ attack relation. Next, we investigate under stable, semi-stable, preferred, grounded and ideal semantics the outputs of logic-based instantiations that satisfy the postulates. For each semantics, we delimit the number of extensions an argumentation system may have, characterise the extensions in terms of subsets of the knowledge base, and finally characterise the set of conclusions that are drawn from the knowledge base. The study reveals that stable, semi-stable and preferred semantics either lead to counter-intuitive results or provide no added value w.r.t. naive semantics. Besides, naive semantics either leads to arbitrary results or generalises the coherence-based approach initially developed by Rescher and Manor (1970). Ideal and grounded semantics either coincide and generalise the free consequence relation developed by Benferhat, Dubois, and Prade (1997), or return arbitrary results. Consequently, Dung’s (1995) framework seems problematic when applied over deductive logical formalisms. (shrink)

Background: It is essential to pay attention to and respect the dignity of nurses to maintain them in their profession while they deliver skilled nursing care. Little is known, however, about how a sense of dignity influences the practitioner. Objectives: The purpose of this study is to describe nurses’ experiences of threats to their dignity occurring within clinical settings, which generates an intention to leave clinical practice. Research design and method: The study was performed using qualitative content analysis. The participants (...) were 21 clinical nurses with work experience who were employed across a range of clinical posts. Data obtained from the semi-structured interviews were analyzed and interpreted using a content analysis approach. Ethical considerations: The study was approved by the Ethics Committee of Tabriz University of Medical Sciences and was guided by the ethical principles of voluntary enrolment, anonymity, privacy, and confidentiality. Findings: Within data analysis, three main themes and nine categories were extracted as follows: (1) lack of professional pride (physician’s dominance, intangible nature of nursing, and negative attitude toward clinical nurses); (2) oppressive work environment (high workload, disrespect, discrimination, and lack of support); and (3) suppression of progressivism (lack of appreciation and attention to meritocracy). Discussion: Almost all of the participants have experienced some degree of disrespect and violation of dignity. In general, attempts made by the participants to show their objection to the threats and to support and protect their dignity have not been effective and in fact made them more inclined to leave the clinical work. Conclusion: According to the views of nurses themselves, identification of the factors threatening nurses’ dignity can be one of the appropriate solutions for the broader and deeper investigation of this phenomenon and can help promote and support nurses’ dignity and their retention within the profession. This essay examines how Brazilian literature has broached changes in the country’s political and social scenario since 2013. Literary production has not only considered socio-political upheavals such as the 2013 protests, the 2016 impeachment of President Dilma Rousseff, and, more recently, the assassination of Rio city council member Marielle Franco as well as the 2020-21 COVID-19 pandemic. Literature has also expanded the signification of “democracy,” broadening the democratic lexicon by employing a language of both demands and entitlements and dispute. By (...) creating a democratic imaginary that oscillates between vindication and exigency, contemporary Brazilian literature delineates the tensions inherent to present-day democratic culture, poised between expanding rights and what scholars have called a "democratic erosion."Keywords: Democracy. BPA Production and Associated Health Risks Bisphenol A is a chemical agent found in many everyday products, including canned goods and plastic food containers. BPA exposure is linked to a variety of adverse health effects, such as obesity and diabetes. To protect the public's health — especially the health of vulnerable fetuses, infants, children, and pregnant women — BPA regulations should encompass products intended specifically for these populations. Even with tremendous public outcry against the use of BPA, current federal restrictions (...) do not reach far enough. The U.S. Food and Drug Administration recently banned the use of BPA in children's sippy cups and baby bottles, but failed to address its inclusion in canned infant formula or plastic tableware.BPA production began in the 1950s to make epoxy resins and was later employed to create polycarbonate plastic products. Laws and policies are vital tools in preventing outbreaks and limiting the further spread of disease, but they can vary in content and implementation. This manuscript provides insight into challenges in responding to recent vaccine-preventable disease outbreaks by examining legislative changes in California, policy changes on certain university campuses, and the laws implicated in a measles outbreak in Minnesota.
